We request the courtesy of a 24 hour notice in the event an appointment needs to be cancelled or rescheduled.
The following fee will be applied for no show, appointments cancelled or rescheduled within 24 hours of your original appointment.

• 1. For procedures less than $50, the cost of the procedure will be applied.
• 2. For procedures from $500-$200, a $50 fee will be applied.
• 3. For procedures more than $200, a $25% or the cost of procedure will be applied.

Appointments booked same day of service will be assessed a no show fee should cancelation become necessary.

For treatments that are pre-paid, treatment will be forfeited without 24 hour notice of cancellation.

Hereby authorize Affordable Laser to charge my credit card as described above for a missed appointment.

I have read this entire agreement and understand that I will be held fully responsible for its terms and charges.  I agree not to charge back Affordable Laser, as long as I receive the services that are entitled to e and guidelines are followed for my rescheduling and cancellation of appointments.

Twenty-four hour notice is required for all rescheduling and Cancellations.
